Important Disclaimer for Patient Portal Use

Thank you for using our patient portal! We're committed to providing a secure and convenient way for you to manage your health information. However, to ensure your safety and the best possible care, please read and understand the following guidelines carefully.

No Medical Practice Through Portal Messaging: Our healthcare providers do not provide medical diagnoses, treatments, or advice through the patient portal's messaging feature. This tool is not intended for active medical treatment requests or consultations between patients and providers. For any medical concerns, please schedule an appointment or contact our office directly.

"Patient Passport" is Elation Health’s secure patient portal, designed specifically as a convenient, patient-centered tool to give individuals 24/7 access to their health information — a secure, centralized digital space where all your important health information travels with you across providers and over time. Just like a real passport stamps your travels, Patient Passport records and organizes your healthcare journey so you (and other providers) can easily reference your full history.

Important Note About Messaging in Patient Passport:
In accordance with our commitment to providing you the highest quality care, we do not allow patients to send direct messages through Patient Passport. This helps ensure we never miss your important questions or concerns. We prioritize dedicated time with your providers and appropriate support from our medical assistants and administrative team through scheduled visits or phone calls. You will, however, be able to reply to any messages we initiate with you when needed. Thank you for understanding and for helping us serve you in the best way possible.

Messaging Response Times: If you send a message through the portal, please know that responses may not be immediate—we appreciate your patience! Our team reviews messages during regular business hours, but for any urgent needs or immediate attention, please call:

Emergencies: The patient portal is NEVER appropriate for emergency medical situations. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately.

Additional Notes for Your Protection:

  • While we take every step to protect your privacy in line with HIPAA regulations, remember that no online system is completely risk-free. Please use strong passwords and avoid sharing your login details.
  • The information in the portal is for your personal use and reference only—it is not a substitute for professional medical advice from your provider.
  • We may update this disclaimer or portal features from time to time; continued use means you agree to any changes.
